Onesource came to me wanting to create a health guide that they could give to their patients. They wanted something small enough that it wasn't cumbersome carrying it around with them but large enough to be able to read. Since most of their customers are elderly I designed up a folded 5"x7" Booklet. This booklet is designed to hold all of the health information and appointments in it that the patient will need for that year. It also has a 12 month calendar in it to jot down appointments they will have in the future. Onesource loved it and has since implemented it with all of their locations.
